Duplicate official transcripts are $3 for each unofficial transcript and $20 for each official transcript.

Send your payment and transcript request in writing to:

Please include your name, name at graduation, date of graduation, name and address of the institution where the official transcript is being sent, your current address, and transcript-request details (e.g., "Please send me 2 unofficial transcripts ($3 each = $6) and send institution listed above 1 official transcript ($20), total=$26)." Make checks payable to: Hurley School of Nursing Alumni Association. Credit cards are also accepted. (Please e-mail jmcderm1@hurleymc.com for further details.)

Diploma Requests

Duplicate diplomas are $50 each.

Please include the following with your request:

Current full name
Current address
Current city, state, zip code
Contact #
Graduation date (mm/dd/yyyy) if known
Name desired on diploma (please print legibly)

Name must be the same as at the time of graduation.

No faxes, emails, or credit cards accepted for duplicate diploma requests. Make checks payable to: Hurley Medical Center. Please allow 3-4 weeks to receive your duplicate diploma.

School Pin

Before ordering your replacement school pin from Herff Jones Co., you must request that a Replacement Pin Request Form be sent to you. Send your request for a Replacement Pin Request Form in writing to:

Once Jennifer McDermitt has received the request form, you may then place your order for a replacement pin with them.
